Request for Maintenance Repairs
I, the undersigned, am requesting repairs and maintenance to be done to my unit as described below. I give the office, maintenance staff, and any approved contractor permission to enter my unit premises between 9:00 am and 5:00 pm, whether I am home or not, in order to complete the requested repairs.
